Project description
: 

The health of medical students is a public health priority, as it has a direct impact on the quality of care and patient satisfaction. Medical school exposes students to constant pressure and a very heavy workload in university hospitals. These conditions make it difficult to adopt healthy behaviors, leading to a decline in students’ quality of life and their physical and mental health. The latest national survey reports that 52% of medical students exhibit symptoms of anxiety and 21% have suicidal thoughts. Medical students also experience sleep disorders and lead very sedentary lifestyles. This reality has profound repercussions, not only on the students themselves but also on the quality of care they will provide as future professionals. 

In light of these challenges, health-based empowerment approaches—aimed at strengthening students’ autonomy and capacity for action—appear promising. The PROMESS project, developed by the RESHAPE laboratory (University of Lyon 1), designed and implemented individual modules on stress management, sleep, and physical activity as part of a randomized controlled trial involving medical students. The students expressed a very high level of satisfaction with the modules.

The HEALTH-PRIME project builds on the PROMESS project and aims to promote sustainable institutional integration within medical schools and enable the implementation of this type of program on a national scale. It will explore the active processes of this health promotion and education program and carefully map out the short-, medium-, and long-term benefits for students and experts (general practice residents specifically trained in the program).

This project will provide a preventive intervention program focused on stress management, sleep, and increased physical activity, tailored to the specific needs of college students. In addition, it will identify the conditions necessary for the sustainable implementation of a health promotion and education program.

It consists of several stages, some of which have already been completed:

- Phase 1: Identification of students’ needs (focus groups) and co-development of the intervention with medical students, experts from the pilot study (PROMESS), and project staff—completed (2026).

- Phase 2: Implementation of the randomized controlled trial—to come.

• Design: A single-center, parallel-arm (1:1:1), randomized controlled trial.

• Population: Volunteer students enrolled in their fourth year at the Lyon-Est and Lyon-Sud medical schools.

• Intervention: A comprehensive intervention focused on stress management, improving sleep, and promoting physical activity.

• Outcome measures: Empowerment, academic performance, sense of competence, quality of life, psychophysiological stress, sleep quality, level of physical activity, sedentary behavior, anxiety, depression, burnout, substance use.

- Phase 3: Evaluation of the program’s effectiveness and its impact—to be determined.

- Phase 4: Large-scale rollout of the program—to be determined.
